Treatment Plans

Treatment plans outline each resident's goals and individual needs, and are a central aspect of the program. Monthly or bi-weekly progress reports are completed for each resident.

The components of a treatment plan include:

  • Diagnosis - A physician will evaluate each resident to determine if a mental health or medical diagnosis is warranted. These diagnoses provide treatment staff and individuals involved in aftercare an accurate picture of the resident's needs and treatment possibilities.
  • Objectives and Goals - Each resident is asked to attain particular goals and objectives. These goals and objectives are measured and reported in percentages of attainment. Results are written in a monthly progress report and shared with parents, social workers, and residents.
  • Priority Problems - As part of the goal-setting process, each resident and her significant others work with treatment professionals to identify priority problems that the resident must work on during her stay at TP.
  • Educational Plan - Each resident's prior credits, earned credits, and current subjects and grades are reported.
  • Progress Narratives - Information from the education, therapy, and nursing staff regarding individual progress in each area.
  • Current list of medications.
  • Aftercare plan - Specifics of any aftercare plans are reported.
  • Anticipated graduation date.
